The Governor's Proclamation announcement on May 24, 2024 included a provision (Section 13) that waives the fee for a certified copy of a vital record if that record was lost, destroyed, or rendered illegible as a result of the severe storm that occurred on May 20, 2024 and continued thereafter. 

 

 As of May 24, 2024, the following counties have been included in the most recent Disaster Emergency:  

Adair, Adams, Black Hawk, Buena Vista, Butler, Calhoun, Cass, Cedar, Cerro Gordo, Clay, Clinton, Dubuque, Fayette, Franklin, Hancock, Hardin, Harrison, Humboldt, Iowa, Jackson, Jasper, Johnson, Keokuk, Kossuth, Marion, Marshall, Mills, Montgomery, Muscatine, O'Brien, Page, Palo Alto, Polk, Pottawattamie, Scott, Shelby, Story, Tama, Union, Warren, Washington, and Wright.

On the application for a certified copy, the purpose of the request should indicate that the record was lost, damaged or destroyed due to severe storm, and the fee is No Fee.  It should be recognized that the address of the individual requesting or receiving the copy should reasonably be within the counties where the severe storm occurred, or that the record was stored in a location within the counties where the severe storm occurred. 

The proclamation shall expire on June 27, 2024 at 11:59 pm unless sooner terminated or extended in writing by the Governor.
