Updated Disaster Proclamation - Replacement Vital Records

ANNOUNCEMENT

Governor's Proclamation

 

 

 


 

The Governor's Proclamation announcement on June 22, 2024 included a provision (Section 12) that waives the fee for a certified copy of a vital record if that the record was lost, destroyed, or rendered illegible as a result of the severe storms and widespread flooding that occurred on June 21, 2024 and continued thereafter.


 

As of June 27, 2024, the following counties have been included in the most recent Disaster Emergency: Adair, Adams, Buena Vista, Butler, Calhoun, Cedar, Cherokee, Clay, Dallas, Franklin, Hamilton, Hancock, Harrison, Humboldt, Jasper, Iowa, Jackson, Kossuth, Marshall, Mitchell, Montgomery, Muscatine, Polk, Pottawattamie, Poweshiek, Shelby, Story, Tama and Wright Counties. 


 

On the application for a certified copy, the purpose of the request should indicate that the record was lost, damaged or destroyed due to severe storm or flooding, and the fee is No Fee.  It should be recognized that the address of the individual requesting or receiving the copy should reasonably be within the counties where the severe storm occurred, or that the record was stored in a location within the counties where the severe storm occurred.

The proclamation shall expire on July 26, 2024 at 11:59 pm unless sooner terminated or extended in writing by the Governor.
