Partner's Program
The Partner’s Program exists to enable clients who do not require skilled services but need services and or/one-time visits to help them benefit from health care support to maintain their independence in the community. The cost for this service is $30/visit and is typically done in the office setting.
Services that qualify but are not limited to:
1) Injections (excluding allergy injections)
2) Medication management and/or education
3) Labs
4) PT/INR
5) Diabetic support
6) Foot soaks/nail care
7) Other
