Home Health Manager
The Home Health Manager (HHM) is responsible for the direct line management of the patient care processes. The HHC develops and implements referral systems and practices, and facilitates clinical management of patients receiving care. The HHM oversees all aspects of service delivery, including patient referral, assignments, care plan development, ongoing patient care, documentation, and customer satisfaction with services. The HHC assists with agency customer service and communications, including physician communication. The HHC participates in development of practice standards, including measurement and trending of patient outcomes related to care, and maintaining services under Federal and State requirements.
